In our case unchecking SmartDefense option "add received header when
forwarding"
seems to have solved the problem.

Every so often the firewall sends a 550 Mailbox unavailable response on
inbound mail and logs it as content security - access denied. If the user
resends it will work. This started after we upgraded from 4.1 SP5.
Using a Sniffer I found this occurs when the firewall waits about a minute
after the connection is opened before sending it's 220 message. If firewall
responds quickly the process succeeds.
