I had the same problem when I upgraded to ipso 3.8.1 from 3.7.1
I tried everything and the solution was to do a rollback to 3.7.1
Do you have floodgate installed?

Is this a first time install or it worked before and now it just stopped
working. Sounds like a network issue. Do you get the can't connect to port
18191 error message? Can you ping the firewalls with the policy unloaded?

This is an unusual setup. You'll have to describe your physical setup such
as cables and hubs between the three boxes.
