Try this, stop smtpd:
svc -d /service/qmail-smtpd
change to directory where your smtpd resides:
cd /var/qmail/boot/qmail-smtpd
Increase log level to 3:
env LOGLEVEL=3 ./run
then again telnet to your server on port 25 from another shell/machine
and see the output of `run`.
Report any error if you can't detect any error.

PS. Which one is true:
~/control/locals.cdb or ~control/locals
~/control/rcpthosts.cdb or ~control/rcpthosts
Both are correct. Whenever you make a change in locals or rcpthosts, you
run make. This creates a database file in .cdb file keeping the prefix
of file same.
