In my configuration  my server  is  qmail-ldap  I install   simscan  because faster  and only scan virus  for intermail mail   and  I install   Smtp gateway  with postfix+    spamassasin+amavisd+clamav   for   incoming mail from internet,   and You can build  Smtp gateway with qmail

qmail-scanner is slow. simple as. If you are saying that your qmail-ldap server is slow then you are doing something wrong.
I use qscanq which is very quick.
