First you patch a fresh qmail installation with the Qmail-LDAP
patch and after that you apply the Qmail-LDAP/Controls patch.
Note that the Qmail-LDAP/Controls patch is version dependent
on the Qmail-LDAP patch!

No. The author(s)/Maintainer(s) of the Qmail-LDAP patch
thinks the Qmail-LDAP/Controls patch is a bad idea, so it won't
be included...

With the latest (i.e. development version - 2.3) of phpQLAdmin,
it should be easier. I'm well aware that documentation sucks,
but what is is in the docs/ directory. If you have read
_all_ of them, then try the phpqladmin list, maybe we
can help you with specific problems there...
