I would suggest you to setup spamassassin 3.1.7 . I  have been working for past six months and its workes really good for me under the circumstances mentioned by you .you can get the same in search.cpan.org. search for spamasssin and integrate with qmail using qmail scanner and then change tcp.smtp file accoringly and see that the same version is clubbed by firing the command  /var/qmail/bin/qmail-scanner -v .
