There are several antiviruses available for qmail, but integration in the queue
process is not always easy.
I personally think that ClamAV is a very good antivirus (and the fact it comes
with no fees is another good thing), I use it and no viruses pass in my 
mailboxes.
It's "on the wire", updates are automatically done by freshclam and it performs
mime unpacking and scannin without external programs.
