Another such variation on the theme is now available at

    http://alkemio.org/software/greetdelay/index.html

Blurb:

greetdelay is a program to introduce a small delay before an SMTP
greeting. It can also optionally enforce RFC 2821's recommendation
that SMTP clients not send any commands before receiving the
    greeting message.

After installing, change

    tcpserver 0 25 qmail-smtpd

to

    tcpserver 0 25 greetdelay qmail-smtpd

. By default, if greetdelay receives any input from the client before
the time limit passes (violating RFC 2821's recommendations), it will
exit without running qmail-smtpd.

I've tested it under OpenBSD 4.0 and Debian GNU/Linux 3.1. I'd love
to hear reports from others.
