I am using Jon Lewis' program: http://www.lewis.org/~jlewis/smtp-delay.c

We have seen a DRASTIC reduction in spam. We've gone from our 8 mail
servers being 3/4 loaded to them being about 1/4 loaded, and so far no
false positives. Amazing!

I gave this a try with a delay of 30 seconds, which was widely
suggested. It definitely did kill spam, but I ran into a problem with
it. Emails from paypal/ebay were not coming in (I have clients that
use paypal accounts and it was fairly important they get those emails).
Also charter.net had problems as well. Those are just the ones that
I was able to diagnose and find, could be others for all I know.
