You can put the program wherever you like, depending on how your system is organized and how much you care about organization. I also don't have much of anything in /usr/local.

Like rblsmtpd, it sits in front of qmail-smtpd and runs it only if certain criteria are met, so you can deduce that it should be chained before qmail-smtpd in your run script.

(3) How do I know that it is blocking spam? Is there a log or so?

Yes, grep for "greetdelay" in your logs.
