Contact the author of the patch you're using that adds that response to
qmail-smtpd:

Running 1.03 with jms combined patch 1.6c.

... "jms" probably knows, or should.

Oh, it's 1.6b that I'm using. I see a few references in the qmail-1.03-jms1.6b.patch file that note 554 - but yeah,
that's all I get from it.
