Sadly, no.

I looked in the qmail-smtpd.c but did see anything (not that I know what
I'm looking for to start with)

I ended up patching qmail-smtpd.c such that it logs it's decisions for all mails. That way I have a better idea of what it's up to (very useful when I added qmail-dk to the mix).
