You are correct, this qmail server has a public IP address. That would depend on where mail is being sent from. Mail is coming from Internet as well as inside. This machine is a smart relay for a few other internal qmail servers. If your goal is to have this qmail server as a kind of multiplexer that receives mail and redirects it to the "real" servers, you would also need to add those domains to control/rcpthosts.
