If it was actually qmail doing the delivery, the most likely culprit would be full disk or filesystem out of inodes.

However, that's not qmail doing the delivery. It's vdeliver -- that's part of vpopmail, isn't it? Ask the vpopmail mailing list.

Actually, vdeliver is part of VMailMgr, but the advice is essentially the same: ask the vmailmgr mailing list.
