Contact the author of the patch you're using that adds that response to
qmail-smtpd:
Running 1.03 with jms combined patch 1.6c.
... "jms" probably knows, or should.

John, if you're reading, any ideas on where that 554 is coming from?
nope. the only place the word "transaction" shows up in the source
code after any of my patches is "503 no auth during mail transaction
(#5.5.0)", which comes from the AUTH patch that i started with,
version 0.31 on http://members.elysium.pl/brush/qmail-smtpd-auth/ ...
so if you're seeing a message with the literal words "transaction
failed", it's not coming from my patch.
sorry it took so long to reply, the past several months have been
really hectic for me, and i'm just now getting around to playing
catch-up with mailing list traffic.
Haa haa, holy crap I'd completely forgotten about that issue . . . must
have resolved it self like so many plumbing leaks I have . . . Anyways
